Annual Forms
Each year, Aidan families are required to complete and submit updated forms for the upcoming school year. These forms help ensure we have current health information, permissions, and required documentation for every student. Please note: all forms for the 2026-2027 school year are due Friday, August 28.
Please complete all forms that apply to your child and submit them according to the instructions below. Existing, non-expired health forms currently on file with Aidan will be transferred to our new system. Families do not need to resubmit these forms unless they have expired, require updates, or are specifically requested by the school.

Aidan is no longer using Magnus Health for annual forms.
All completed forms should be submitted directly to our new Aidan’s Forms email address: forms@aidanschool.org.
